The PDF attached to this post explains and shows how OSI implemented fatigue loss for players as they walk and run around in the world (either mounted or not).
The presented document does not include the mobile's fatigue gain code.

NOTE: the infamous "all follow me"-bug also exists in the demo and is a feature (not a bug) of the NPC AI code, I'll make a post about this another day
